Tiotropium is used to treat bronchospasm caused by chronic obstructive pulmonary disease (COPD), such as chronic bronchitis and emphysema, over a long period of time.



Tiotropium is a long-acting antimuscarinic drug that is also known as an anticholinergic drug. It has a similar affinity for M1 to M5 muscarinic receptor subtypes. It has pharmacological effects in the airways by inhibiting M3-receptors in the smooth muscle, resulting in bronchodilation.


Dosage & Administration

With the device, the recommended dose of Tiotropium Dry Powder Inhaler capsule is 18 mcg (1 capsule) once daily.

The Tiotropium Dry Powder Inhaler capsules’ contents are only for oral inhalation and should be used with the device.

Geriatric, hepatically impaired, and renally impaired patients do not require any dosage adjustments.



Tiotroplum (14.4 mcg intravenous infusion over 15 minutes) and cimetidine 400 mg three times daily or ranitidine 300 mg once daily were tested in an interaction study.



Patients with a history of hypersensitivity to atropine or its derivatives, including ipratropium, or any component of this product should not take tiotropium.


Side Effects

Dry mouth was the most commonly reported adverse drug reaction. Dry mouth was usually mild and went away after a few weeks of treatment. Constipation, increased heart rate, blurred vision, glaucoma, urinary difficulty, and urinary retention were among the other side effects reported in individual patients, all of which were consistent with possible anticholinergic effects.


Pregnancy & Lactation

Pregnancy Classification .There are no adequate and well-controlled studies on Tiotropium in pregnant women. Only if the potential benefit outweighs the risk to the fetus should it be used during pregnancy. During labor and delivery, the safety and effectiveness of Tiotropium have not been studied.


Precautions & Warnings

Tiotropium, as an anticholinergic, may exacerbate symptoms and signs of narrow-angle glaucoma, prostatic hyperplasia, or bladder-neck obstruction, and should be used with caution in patients with any of these conditions.
