Indomethacin

Indications

Hheumatoid arthritis, ankylosing spondylitis, and (degenerative joint disease), gout, acute rheumatism (bursjlis, synovitis. tendinitis).

Pharmacology

Indomethacin is an NSAID with antipyretic and analgesic properties. Numerous pharmacological investigations have demonstrated Indomethacin’s potent anti-inflammatory, analgesic, and antipyretic effects. Unlike corticosteroids, Indomethacin has no impact on the pituitary gland or the adrenals. Indomethacin is rapidly absorbed after oral treatment, with peak plasma levels occurring within 2 hours. Two-thirds of this quantity is eliminated in the urine, with the remaining excreted in the feces. In 24-48 hours, about 90% of a single dosage is eliminated. Indomethacin has been shown to be effective in the treatment of rheumatic illnesses in a number of controlled clinical trials. It relieves pain, decreases joint swelling and edema, and increases joint mobility.

Dosage & Administration

Rheumatoid arthritis, ankyloslng spondylitis, and osteoadhritis (degenerative joint disease): The iniiial dose is once daily of Reumacap SR capsule. lt severe then twice daily is required. Acute attacks of gout may control with a dose of 75 mg sustained release capsule twice daily. Acute non-articular rheumatism (bursitis, synovitis, tendinitis): ‘1-2 Reumacap SR capsules daily according to the severity of this case.

Interaction

It may interact with anticoagulants, Lithium, diuretics, antihypertensive drugs, dif lunisal and sulfonylureas.

Contraindications

indomethacin is contraindicated in patients with ulcer, gastritis, actlve ulcerative colitis, and it should be used with caution in patients with a history of these disorders. ln these cases, administration of lndomethacin suppositories should be preferred. lt is also contraindicated in the previously hypersensitive patients.

Side Effects

During the first few weeks of therapy, headaches, generally in the morning, and mild vertigo may develop. These effects are generally transitory and resolve with prolonged usage or by lowering the dosage. A few cases of moderate psychological problems were also recorded, however these resolved after a few days of therapy. Large dosages of the medication frequently cause gastrointestinal symptoms such as nausea, vomiting, diarrhoea, epigastric and abdominal discomfort, which diminish when the dose is lowered. Administration of the capsule immediately after meals with an antacid it necessary, to minimize the frequency and the severity of these untoward effects.

Pregnancy & Lactation

Pregnancy: Pregnant women are not advised to use this product since clinical trials are lacking at this time.

Precautions & Warnings

Fieumacap SB capsule should be used with caution with psychiatric problems, epilepsy or parkinsonism since the drug may aggravate these conditions. lt is not recommended for pregnant women, because at the present time clinical studies are insufficient. lndomethacin is not given nomally in neonates except when used to assist closure of a patent ductus arteriosus.
