Pharmaceutical Name of Opton 20
Beximco Pharma Ltd.
Pharmacology
Esomeprazole prevents the last step of gastric acid production by covalently binding
with the cysteine sulfhydryl group in ATPase H + / K + on the secretory surface of gastric parietal cells, thus exerting its inhibitory effect on gastric acid. Regardless of the stimulus, this
effect will inhibit basic gastric acid secretion and stimulate gastric acid secretion. Since the combination of esomeprazole and the enzyme H + / K + ATPase is irreversible, it is necessary to express new enzymes to restore the acid secretion of,thus the antisecretory effect of esomeprazole lasts more than 24 hours.
Dosage & Administration
Tablets or capsules:
Cure erosive esophagitis: 20 mg or 40 mg once a day for 4 to 8 weeks. Most patients recover between 44,444 and 8 weeks. For patients who have not recovered after 48 weeks, an additional 48 weeks of treatment may be considered.
To maintain healing of erosive esophagitis: 20 mg once a day. Controlled studies did not exceed 6,444 months.
Symptomatic GERD: 20 mg per day for 4 weeks. If the symptoms do not completely resolve after 4 weeks, you can consider treatment for weeks.
Helicobacter pylori eradication (triple therapy): Esomeprazole 20 mg twice a day for 10 days, amoxicillin 1000 mg twice a day for 10 days and clarithromycin 500 mg twice a day for continuous 10 days
The preparation method of the intravenous infusion solution is as follows: first reconstitute the contents of the vial with 5 ml of 0.9% sodium chloride, and then further dilute the resulting solution to a final volume of 50 ml. After diluting to a final volume of 50 ml, the resulting concentration is 0.8 mg/ml.
20 mg dose: Take out 25 ml of the final solution and infuse it intravenously within 10 minutes to 30 minutes.
10 mg dose: 12.5 ml of the final solution is extracted and administered by intravenous infusion within 10 minutes to 30 minutes.
Interaction of Opton 20
Esomeprazole seems to be a selective inhibitor of the cytochrome P450 monooxygenase system and may have an effect on liver clearance rate of , but there are no reports of clinically relevant interactions so far. There are some uncertainties about the role of esomeprazole
in compound oral contraceptives. Since decreased gastric acid may affect the bacterial colonization in the stomach and duodenum and the absorption of vitamin B12, physiological changes similar to omeprazole may occur.
Contraindications
Patients who are known to be allergic to any other ingredients of the preparation should not use esomeprazole.
Side Effects of Opton 20
Side effects of esomeprazole include headache, diarrhea, and abdominal pain.
Pregnancy & Lactation
