Indications
This is recommended for the treatment of bacterial infections on the skin. This is very useful for atopic dermatitis, contact dermatitis and infections, eczematoid dermatitis, neurodermatitis, eczema, and anogenital pruritus. It may also be beneficial as an adjuvant during specialized systemic antibiotic therapy for some pyodermas, such as impetigo.
Pharmacology
Bacitracin inhibits bacterial cell wall production and is effective against numerous gram-positive bacteria (including staphylococci, streptococci, corynebacteria, and Clostridia) as well as some gram-negative bacteria (e.g. Neisseria and Haemophilus influenzae). As broad-spectrum antibacterial agents, they are frequently encountered in combination in topical treatments.
Neomycin, an aminoglycoside having a similar antibacterial range to gentamicin, binds to the 30S subunits of the bacterial ribosome, preventing protein synthesis and thereby affecting DNA synthesis. It is effective against several gram-negative aerobes as well as some staphylococci strains.
Dosage & Administration
Dusting Powder: Antibiotic powder is a non- sterile dusting powder and used for superficial skin infections. A light dusting of the powder to be applied to the affected skin area upto 4 times daily.
Interaction
When used with other nephrotoxic medications, bacitracin zinc increases the risk of nephrotoxicity. It is possible that neuromuscular blocking drugs will be more effective.
Other aminoglycosides, bacitracin, cisplatin, vancomycin, amphotericin B, polymyxin B, colistin, and viomycin have additive nephrotoxic and neurotoxic effects. Toxicity is increased when strong diuretics are used. Other medicines’ absorption may be hampered. Acarbose’s impact may be enhanced. Non-depolarizing muscle relaxants may be enhanced. The parasympathomimetic action of neostigmine and pyridostigmine may be inhibited. Patients using bisphosphonates may be at increased risk of hypocalcaemia. When used with anticoagulants, it may cause an increase in INR. Oral typhoid vaccination may be rendered ineffective.
Contraindications
Any of its constituents can cause hypersensitivity.
Side Effects
This medication is generally well tolerated. Allergic responses may develop in rare situations.
Pregnancy & Lactation
This preparation’s safety during pregnancy and breastfeeding has not been proven. As a result, it should only be taken during pregnancy if the possible benefit outweighs the potential danger to the fetus.
Precautions & Warnings
Bacitracin and Neomycin antibiotic powder should not be applied to big open wounds, badly damaged skin, or regions that release considerable amounts of fluid because hard crusts may form. Because this skin ointment includes Neomycin, it should not be used to treat external otitis when the eardrum is perforated due to the risk of ototoxicity.
Patients seeking self-medication with a topical anti-infective for deep or puncture wounds, animal attacks, or severe burns should seek medical advice first. Patients who use the preparations to prevent infection in minor skin injuries (e.g., cuts, scrapes, and burns) should be advised to discontinue the topical anti-infective preparation and consult a physician if the condition persists or worsens; it should not be used for more than one week unless directed by a physician.
