Indications of Myocard 5 Tablet

Myocard is indicated for– Mild to moderate Hypertension, management of Heart Failure, Angina Pectoris, secondary prevention of Myocardial Infarction (MI).


Pharmacology of Myocard 5 Tablet

Pharmacodynamic Properties: Bisoprolol selectively blocks catecholamine stimulation of β1-adrenergic receptors in the heart and vascular smooth muscle.This result in a reduction of heart rate, cardiac output, systolic and diastolic blood pressure.

Pharmacokinetic Properties:

  • Absorption: The bioavailability of Bisoprolol from film coated tablets is about 90%.
  • Distribution: Only 30% of the Bisoprolol in the blood is bound to plasma proteins.
  • Metabolism: Approximately 50% of the dose is metabolized by CYP3A4.
  • Excretion: The plasma elimination half-life is approximately 9-12 hours.


Dosage & Administration of Myocard 5 Tablet

In some patients, 2.5 mg may be an appropriate starting dose but the usual starting dose is 5 mg once daily. If the antihypertensive effect of 5 mg is inadequate, the dose may be increased to 10 mg and then, if necessary, to 20 mg once daily.

Patients with Renal or Hepatic Impairment

  • In patients with hepatic impairment (hepatitis or cirrhosis) or renal dysfunction (creatinine clearance less than 40 ml/min), the initial daily dose should be 2.5 mg.

Missed Dose

  • If a dose isn’t taken at scheduled time, administer it as soon as possible on the same day.


Interaction of Myocard 5 Tablet

Hypoglycemic agents, phenothiazines and various antiarrhythmic agents – Such medicine interactions can have life-threatening consequences.



Bisoprolol is contraindicated in patients with known hypersensitivity to Bisoprolol, patients with asthma, bronchitis, chronic respiratory diseases, second and third degree heart block, bradycardia less than 50 beats per minute, peripheral vascular disease, uncontrolled cardiac failure, pregnancy or lactation.


Side Effects of Myocard 5 Tablet

Fatigue, nausea, vomiting, diarrhea, constipation, dizziness, headache, feeling of coldness or numbness in the extremities, hypotension.


Pregnancy & Lactation

Pregnancy Category of C. Bisoprolol is contraindicated during pregnancy or lactation as the safety of Bisoprolol during pregnancy and whilst breast-feeding has not been established.


Precautions & Warnings

Heart failure, bronchospasm, renal & hepatic impairment, diabetes mellitus, ongoing desensitization therapy, first degree AV block, prinzmetal’s angina, peripheral arterial occlusive disease, general anesthesia.
