Description
Indications
When the response to diet and other non-drug measures is insufficient, atorvastatin can be used as a dietary supplement to reduce total cholesterol, low-density lipoprotein cholesterol, apolipoprotein B (ApoB) and triglycerides in the following cases High level.
- Reduces total cholesterol and low-density lipoprotein cholesterol in patients with heterozygous and homozygous familial hypercholesterolemia.
- Reduces elevated cholesterol and triglycerides in patients with mixed dyslipidemia (Fredrickson type Ia and Ib).
- Is used to treat patients with elevated serum triglyceride levels in hypertriglyceridemia (Fredrickson type IV).
- Is used to treat patients with abnormal beta lipoproteinemia (Fredrickson type III).
- Reduces cardiac ischemic events in asymptomatic patients or patients with mild to moderate symptomatic coronary artery disease with elevated levels of low-density lipoprotein cholesterol.
- Reduces total cholesterol and low-density lipoprotein cholesterol levels in patients with hypercholesterolemia associated with or aggravated by diabetes or kidney transplantation.
Pharmacology of Locol 20
Atorvastatin is a selective inhibitor of HMG-CoA reductase. This enzyme is the rate-limiting enzyme responsible for the conversion of HMG-CoA to mevalonate, a precursor of sterols, including cholesterol. Atorvastatin lowers plasma cholesterol and lipoprotein levels by inhibiting HMG-CoA reductase and cholesterol synthesis in the liver and increases the number of hepatic LDL receptors on the cell surface for enhanced uptake and catabolism of LDL.
Dosage & Administration
Primary hypercholesterolemia and combined hyperlipidemia-
- Adults: Usually 10 mg once daily; if necessary, may be increased at intervals of at least 4 weeks to the max. 80 mg once daily.
- Child (10-18 years): Initially 10 mg once daily, increased if necessary at intervals of at least 4 weeks to the usual max. 20 mg once daily.
Familial hypercholesterolemia-
- Adults: Initially 10 mg daily, increased at intervals of at least 4 weeks to 40 mg once daily; if necessary, further increased to max. 80 mg once daily (or 40 mg once daily combined with anion-exchange resin in heterozygous familial hypercholesterolemia).
- Child (10-18 years): Initially 10 mg once daily, increased if necessary at intervals of at least 4 weeks to the usual max. 80 mg once daily.
Prevention of cardiovascular events-
- Adults: Initially 10 mg once daily adjusted according to the response.
Interaction of Locol 20
Fluconazole is a drug that is well tolerated. Nausea, vomiting, abdominal pain, diarrhea, headache, and skin rash are the most common Fluconazole side effects.
Contraindications
Patients who are hypersensitive to any component of Locol 20 should avoid taking it. Active liver disease or unexplained chronic transaminase increases are contraindicated with atorvastatin. It’s also not recommended for patients who have had a significant reaction to HMG-CoA reductase inhibitors in the past.
Side Effects of Locol 20
Locol 20 is well tolerated by most people. Constipation, flatulence, dyspepsia, and abdominal pain are the most common Atorvastatin side effects. Infection, headache, back pain, rash, asthenia, arthralgia, and myalgia are some of the other side effects.
Pregnancy & Lactation
Pregnancy: Locol 20 is contraindicated during pregnancy. The safety of pregnant women has not been determined. Controlled clinical trials of atorvastatin have not been conducted on pregnant women. Rare reports of birth defects have been received after exposure to HMGCoA reductase inhibitors in utero. Animal studies show reproductive toxicity. Treatment of mothers with atorvastatin may reduce the level of mevalonate in the fetus, which is a precursor to cholesterol biosynthesis. Atorvastatin should not be used in women who are pregnant, trying to become pregnant, or are suspected of becoming pregnant. Treatment with Locol 20 should be discontinued during pregnancy or before it is determined that the woman is not pregnant.
Lactation period: It is unknown whether atorvastatin or its metabolites are excreted in human milk. In rats, the plasma concentrations of Locol 20 and its active metabolite are similar to those in milk. Because of the potential for serious adverse reactions, women taking atorvastatin should not breastfeed their babies. Locol 20 is contraindicated during lactation.
Precautions & Warnings
Liver effects: Liver function tests should be performed before the initiation of treatment and periodically thereafter. Atorvastatin should be used with caution in patients who consume substantial quantities of alcohol or have a history of liver disease. Atorvastatin therapy should be discontinued if markedly elevated CPK levels occur or myopathy is diagnosed or suspected.
