Indications of Exovate N 25 Cream
E x o v a t e® N cream is indicated in more resistant dermatoses such as recalcitrant eczemas and psoriasis (excluding widespread plaque psoriasis) where secondary bacterial or candidal infection is present, suspected or likely to occur, as when using occlusive dressings.
Pharmacology of Exovate N 25 Cream
Clobetasol Propionate is a highly effective corticosteroid. It’s used to treat severe inflammatory skin conditions including eczema and psoriasis that haven’t responded to milder corticosteroids. Neomycin Sulphate is an aminoglycoside antibiotic that is used to treat bacterial infections. Nystatin is an antifungal that works by interfering with the cell membranes of fungi and yeasts. In general, the mechanism of topical steroids like Clobetasol is unknown. Clobetasol Propionate, on the other hand, is a potent corticosteroid with anti-inflammatory properties. Clobetasol Propionate’s main effect on skin is a nonspecific anti-inflammatory reaction, which is caused by vasoconstriction and a reduction in collagen production. Neomycin suppresses protein synthesis by binding to the ribosomal 30s and 50s subunits of sensitive bacteria. Neomycin also causes a misreading of the mRNA template’s genetic coding, resulting in the incorporation of erroneous amino acids into the developing polypeptide chain. Nystatin works by attaching to sterols in the fungus’ cell membrane, causing a shift in membrane permeability that allows intracellular components to leak.
Dosage & Administration of Exovate N 25 Cream
Adults: Apply sparingly to the affected area once or twice daily until improvement occurs. In very resistant lesion, especially where there is hyperkeratosis, the anti-inflammatory effect of this preparation can be enhanced (if necessary) by occluding the treatment area with polythene. Treatment should not be continued for more than 7 days without medical supervision. If a longer course is necessary, it is recommended that treatment should not be continued for more than 4 weeks without the patient’s condition being reviewed.
Elderly: This preparation is suitable for use in elderly. Caution should be exercised in cases where a decrease in renal function exists and significant systemic absorption of Neomycin Sulphate may occur.
Children: This preparation is suitable for use in children (2 years and over) at the same dose as adults. A possibility of increased absorption exists in very young children, thus this cream/ointment is not recommended for use in neonates and infants (younger than 2 years).
Interaction of Exovate N 25 Cream
Following considerable systemic absorption, neomycin sulphate can enhance and extend the respiratory depressive effects of neuromuscular blocking drugs. However, if administered as directed, systemic exposure to Neomycin Sulphate is predicted to be low, and medication interactions are unlikely to be substantial. There have been no reports of dangerous interactions with the usage of Clobetasol Propionate or Nystatin.
Contraindications
Rosacea, acne vulgaris, and perioral dermatitis are contraindicated, as are primary cutaneous viral infections (e.g., Herpes simplex, chicken pox), and hypersensitivity to the preparation.
Side Effects of Exovate N 25 Cream
As with other topical corticosteroids, continuous use of a high quantity or treatment of broad regions can result in enough systemic absorption to induce hypercortisolism-like symptoms. The impact is more probable in infants and youngsters, as well as when occlusive bandages are utilized. Prolonged and extensive use of highly active corticosteroid preparations may result in local atrophic changes in the skin, such as thinning, striae, and dilatation of the superficial blood vessels, especially when occlusive dressings are employed or skin folds are involved. Topical steroids have been linked to pigmentation alterations and hypertrichosis.
Pregnancy & Lactation
There is minimal evidence to support the use of topically administered Neomycin during pregnancy and breastfeeding. However, because Neomycin in the maternal blood might pass the placenta and provide a potential risk of foetal harm, the preparation is not advised for use during pregnancy and breastfeeding. Clobetasol Propionate’s safety in nursing women has not been verified.
Precautions & Warnings
Long-term continuous topical treatment should be avoided as much as possible, especially in babies and children, because adrenal suppression can develop even in the absence of blockage. If applied to the eyelids, care must be taken to ensure that the preparation does not enter the eye, since this might lead to glaucoma. Clobetasol Propionate’s safety in nursing women has not been verified.
