Hydrochlorothiazide + Triamterene
Indications
This medicine is used to treat high blood pressure. Lowering high blood pressure can help prevent strokes, heart attacks, and kidney problems. This medication is a combination of two “water pills” (diuretics): methotrexate and hydrochlorothiazide. This combination is suitable for people who have low potassium levels or are at risk due to hydrochlorothiazide. It will make you produce more urine, which helps your body excrete excess salt and water.
This medication can also reduce excess fluid (edema) in the body caused by diseases such as heart failure, liver disease, or kidney disease. This can reduce symptoms such as shortness of breath or swollen ankles or feet.
Pharmacology
Triamterene directly inhibits the exchange of Na and K and hydrogen in the distal renal tubules. Hydrochlorothiazide increases the excretion of Na and Cl ions by reducing the reabsorption of electrolytes by the renal tubules, thereby increasing the excretion of water.
Hydrochlorothiazide is a thiazide diuretic. Thiazides affect the mechanism of renal tubular electrolyte reabsorption and directly increase the excretion of approximately the same amount of sodium and chlorine. Indirectly, the diuretic effect of hydrochlorothiazide reduces plasma volume, thereby increasing plasma renin activity, increasing aldosterone secretion, increasing urine potassium loss and reducing serum potassium. Renal aldosterone binding is mediated by angiotensin II, so co-administration of angiotensin converting enzyme (ACE) inhibitors tends to reverse the potassium loss associated with these diuretics.
Dosage & Administration
Hypertension: Initially, 1 tablet (Hydrochlorothiazide 25 mg and Triamterene 50 mg) daily after the morning meal, adjust thereafter according to response. Max: 4 tab daily.
Oedema: Initially, 1 tablet (Hydrochlorothiazide 25 mg and Triamterene 50 mg) bid. Maintenance: 1 tab daily or 2 tab on alternate days. Max: 4 tab daily.
Take this medication by mouth as directed by your doctor, usually once daily in the morning with or without food. It is best to avoid taking this medication within 4 hours of your bedtime to prevent having to get up to urinate.
If you also take certain drugs to lower your cholesterol (bile acid-binding resins such as cholestyramine or colestipol), take this product at least 4 hours before or at least 4 to 6 hours after these medications.
The dosage is based on your medical condition and response to treatment. Use this medication regularly to get the most benefit from it. To help you remember, take it at the same time each day. It is important to continue taking this medication even if you feel well. Most people with high blood pressure do not feel sick.
Interactions
It can reduce the rate of removal of lithium from the kidney. You can antagonize the diuretic effect with NSAIDs, corticosteroids, estrogens, and combined oral contraceptives. The effect increases when combined with other antihypertensive drugs, baclofen and tizanidine. May reduce arterial response to norepinephrine. Increases the response to tubocurarine. Indomethacin causes the risk of acute kidney failure. The use of reboxetine and tacrolimus may increase the risk of hyperkalaemia. Platinum compounds (such as cisplatin) have an increased risk of ototoxicity and nephrotoxicity.
Contraindications
Hyperkalaemia (≥5.5 mEq / L), hypercalcemia, diabetic ketoacidosis, Addison’s disease, progressive renal failure, and worsening liver dysfunction. Use with K supplements and other K-protective medications (including ACE inhibitors).
Side Effects
Nausea, vomiting, diarrhea, dizziness, weakness, low blood pressure, headache, muscle cramps, dry mouth, thirst, allergic reactions, skin rash, metabolic acidosis, pancreatitis. There is very little SLE and photosensitivity.
Pregnancy & Lactation
Category C: Animal studies have shown adverse effects on the fetus (teratogenic or embryo killing or other), and there are no controlled studies on women, or no studies on women and animals. The drug should be given only when the potential benefit justifies the potential risk to the fetus.
Precautions & Warnings
Pre-diabetes or diabetic patients, diabetic nephropathy, gout tendency, history of kidney stones. Liver or kidney failure during pregnancy and breastfeeding.
