Indications of Cardizem-SR 90
The sustained release tablet of Cardizem-SR 90 is used to prevent and treat angina pectoris. It’s used to treat angina pectoris, both classical and vasospastic. It’s also been used to treat essential hypertension (high blood pressure). It’s used to prevent certain types of supraventricular tachyarrhythmias.

Pharmacology
Calcium channel antagonist diltiazem hydrochloride has a modest negative inotropic effect and acts as a peripheral and coronary vasodilator. Diltiazem blocks cardiac conduction in the sino-atrial and atrioventricular nodes, in particular. The activities of diltiazem are as follows:
Direct dilation of coronary arteries and arterioles demonstrated oxygen flow to cardiac tissues. Furthermore, dilation of the peripheral vasculature lowers the systemic pressure of cardiac “afterload,” resulting in less stress and lower oxygen demands in the myocardial tissues.
Antiarrhythmic: The slowed electrophysiological activity across the S-A and A-V nodes is caused by the suppressed influx of calcium ions in cardiac tissues, which does not impact accessory bypass conduction or normal atrial action potential, or intraventricular conduction.
Vasodilation reduces peripheral vascular resistance, making it antihypertensive. More than 90% of an oral dosage is absorbed quickly. The level of protein binding is really high. Within 30-60 minutes, the plasma concentration reaches its peak. It is metabolized in the liver and eliminated in the bile (60 percent).
Dosage & Administration
Mild to moderate hypertension: initially 90 mg or 120 mg twice daily (elderly once daily); up to 360 mg daily may be required (elderly up to 240 mg) daily.
Angina: initially 90 mg or 120 mg twice daily in divided doses may be required (elderly up to 240 mg daily).
Interaction of Cardizem-SR 90
During diltiazem medication, concomitant preventive therapy with short- or long-acting nitrates can be safely provided. When diltiazem is used in conjunction with other medicines that alter cardiac contractility and/or conduction, Diltiazem recommends caution and careful dosage titration.
Contraindications
Patients with a known hypersensitivity to the medicine, sick sinus syndrome, second or third-degree AV block, severe hypertension, acute myocardial infarction, or radiographically proven pulmonary congestion should not take Diltiazem Hydrochloride sustained release tablet.
Side Effects of Cardizem-SR 90
There were reports of bradycardia, sinoatrial block, atrioventricular block, hypertension, malaise, headache, hot flushes, GIT problems, edema, hepatitis, and depression.
Pregnancy & Lactation
There have been no sufficient and controlled studies using diltiazem in pregnant women to date, and the drug should only be used during pregnancy if the potential benefits outweigh the risk to the fetus. Diltiazem is delivered through milk, so women who are taking it should not breastfeed their babies. If diltiazem medication is regarded as necessary in nursing women, an alternative mode of infant feeding should be employed.
